Clinical Information
Evidence-based details for healthcare providers
A valid prescription from a DEA-registered prescriber is required.
Limited to 5 refills within 6 months of the original prescription date.
Subject to state and federal controlled substance regulations.
Use as directed by your healthcare provider. The pellets are inserted by a trained professional during an office visit. If you feel symptoms returning sooner than expected, contact your provider. Do not try to remove or adjust the pellets yourself.
The testosterone pellet dissolves slowly, releasing testosterone into your bloodstream. This raises and maintains healthy testosterone levels, which can improve energy, mood, muscle mass, and sexual function. Anastrozole blocks the aromatase enzyme that turns some testosterone into estrogen. This helps keep estrogen from rising too high, lowering the risk of breast tenderness or mood changes. Most people begin noticing benefits within 2–4 weeks, with full effects in 6–8 weeks. Each pellet lasts about 3–6 months, depending on your metabolism and activity level.
Do not use if you have or may have prostate cancer or male breast cancer, because hormones can make these cancers grow faster. Do not use if you are allergic to testosterone, anastrozole, or any ingredients in the pellets. Do not use if you have uncontrolled heart failure, severe liver disease, or high calcium levels. Not for use in women who are pregnant, may become pregnant, or are breastfeeding (may harm the baby). Not for use in children or teens.
Testosterone may increase the effect of blood thinners like warfarin and could change insulin needs in people with diabetes; close monitoring is needed. Taking steroids such as prednisone at the same time can raise fluid-retention risk. Anastrozole may be less effective if you take estrogen-containing products (birth control pills, topical estrogen). Heavy alcohol use can stress the liver and raise side-effect risk. Always tell your provider about all prescriptions, over-the-counter drugs, vitamins, and herbal supplements, including saw palmetto and DHEA.
Very common (affects more than 1 in 10 people): mild bruising or soreness at the insertion site for a few days. Common (affects 1–10 in 100 people): acne, oily skin, increased red blood cell count, swelling in ankles, mood changes, increased body hair. Less common (1–10 in 1,000 people): breast tenderness, high blood pressure, sleep apnea worsening, changes in cholesterol, headache, hot flashes. Rare but serious: blood clots (leg pain, chest pain), prostate enlargement causing urinary problems, severe liver problems (yellow skin or eyes). Call your provider if you have trouble urinating, severe swelling, or signs of a blood clot.
Store unopened pellets at room temperature (68-77°F, 20-25°C) in a dry place away from direct light. Keep in the original, tightly sealed pouch until insertion. Do not freeze or expose to high heat. Keep out of reach of children. Unused or expired pellets should be returned to the pharmacy or disposed of in a DEA-approved take-back program; do not flush them.
Frequently Asked Questions
How often will I need new pellets?
Most people need a new pellet insertion every 3 to 6 months. Your exact schedule depends on how fast your body uses the hormones and how you feel. Your provider will check blood levels and symptoms to decide the best timing.
Will the insertion procedure hurt?
The area is numbed with a small shot of local anesthetic, so you should only feel brief pressure. Afterward, mild soreness or bruising can last a few days. Using an ice pack and avoiding heavy exercise on the first day can lessen discomfort.
Can I work out right after getting pellets?
Light walking is fine, but avoid intense exercise, swimming, or soaking in a hot tub for 3 to 5 days to let the site heal. Follow any additional instructions from your provider about bandage care.
What if I start feeling symptoms before my next appointment?
Call your healthcare provider. They may check your hormone levels and decide if you need an earlier insertion or an adjustment in dose. Do not try to add over-the-counter testosterone products on your own.
Is weight gain common with testosterone pellets?
Some people notice a few pounds of water weight early on, but many also gain lean muscle and lose fat over time. Eating a balanced diet and exercising regularly can help manage body changes.
Can these pellets cause hair loss or acne?
Higher testosterone can increase oil production, which may lead to acne or speed up male-pattern hair loss if you are already prone to it. Let your provider know if these problems bother you; topical treatments or dose changes may help.
Do I need to stop my other medications when I start pellets?
Usually not, but some drugs—like blood thinners or insulin—may need dose adjustments. Give your provider an updated list of everything you take so they can monitor for interactions.
