Clinical Information

Evidence-based details for healthcare providers

Use as directed by your healthcare provider. Inject only the dose your provider prescribes. If you forget a dose, skip it and wait until your next planned sexual activity. Do not inject more than once in 24 hours and do not exceed three injections per week unless told otherwise by your provider.

PGE1 relaxes smooth muscle and widens blood vessels inside the penis. This increases blood flow, which helps create and maintain an erection. Most men feel the effect within 5–20 minutes after injection. The erection usually lasts 30–60 minutes but may vary with dose and individual response.

Do not use if you are allergic to alprostadil or any ingredient in this product. Do not use if you have penile implants, severe curvature, Peyronie’s disease, or other abnormal penile anatomy (raises risk of injury). Do not use if you are prone to priapism (painful, prolonged erection), such as with sickle cell disease, leukemia, or multiple myeloma. Avoid if you have severe heart disease that makes sexual activity unsafe. Not for use in women, children, or anyone under 18.

Using blood thinners like warfarin or heparin can increase bleeding or bruising at the injection site. Combining with other ED treatments (such as sildenafil or tadalafil) raises the chance of prolonged erections or low blood pressure—do so only if your provider specifically instructs. Medicines that lower blood pressure (alpha-blockers, nitrates) may add to dizziness or fainting. Drinking alcohol shortly before or after the injection can worsen light-headedness.

Very common (affects more than 1 in 10 men): mild to moderate penile pain or burning during or after injection. Common (1–10 in 100): redness, swelling, or small bruise at injection site; prolonged erection lasting 1–4 hours (seek care if it lasts longer than 4 hours); light-headedness. Uncommon (1–10 in 1,000): penile fibrosis (hard lumps), low blood pressure with fainting, itching at injection site. Rare but serious (less than 1 in 1,000): priapism (erection over 4 hours), severe allergic reaction. Most minor effects fade within a few hours; contact your provider if side effects are persistent or severe.

Store in a refrigerator at 36–46 °F (2–8 °C) in the original vial with the cap tightly closed. Keep away from light and do not freeze. If accidentally frozen, discard the vial. Keep out of the reach of children. Dispose of used needles in a sharps container. Return expired or unused medication to a pharmacy take-back program; do not pour down drain or throw in trash.

Frequently Asked Questions

1

How do I give myself the injection safely?

Your healthcare provider will show you the exact injection technique. Always use a new, sterile needle, inject into the side of the shaft (never the head or base), and rotate sites to avoid bruising. Dispose of needles in a sharps container.

2

When will the medication start working and how long will it last?

Most men notice an erection within 5 to 20 minutes. The effect usually lasts 30 to 60 minutes, though this can vary with dose and individual response.

3

What should I do if my erection lasts more than 4 hours?

An erection that lasts over 4 hours may be priapism and needs urgent care. Go to the nearest emergency department or call your provider right away to prevent permanent damage.

4

Can I use PGE1 with Viagra or other pills for ED?

Using both at the same time can raise the risk of very low blood pressure or a prolonged erection. Only combine treatments if your healthcare provider specifically tells you it is safe.

5

Is it normal to feel pain after the injection?

Mild stinging or aching is common and usually fades within a few minutes to an hour. If pain is severe, lasts longer than an hour, or you notice lumps forming, contact your provider.

6

How should I store the vial when traveling?

Keep the vial cold with a small insulated bag and ice pack. Do not allow it to freeze. Use within the time your provider advises and keep it upright to avoid leakage.

Quad-Mix

This compounded injectable medication (often called “Quad-Mix”) contains four active drugs that work together to improve erectile function in adult men. When injected into the side of the penis before sexual activity, the medicine relaxes smooth muscle, widens blood vessels, and reduces outflow so more blood stays in the erectile tissue. A very small volume is used, and the effect starts within about 5–20 minutes and can last 30–90 minutes. Because it is a sterile compound prepared for each patient, strength ratios and vial size may vary based on your prescriber’s order.
