Clinical Information

Evidence-based details for healthcare providers

Use as directed by your healthcare provider. Place one tablet on your tongue and let it fully dissolve before swallowing. If you miss a dose, take it as soon as you remember unless it is almost time for your next dose. Do not take two doses at the same time. Your provider will decide how long you should stay on therapy.

Estradiol is a natural form of estrogen. It attaches to estrogen receptors throughout the body, helping balance hormone levels when your ovaries no longer make enough estrogen. Relief of hot flashes and night sweats often starts within 1–3 weeks. Vaginal dryness may take up to 8 weeks to improve. Effects last about 24 hours, so tablets are taken once daily.

Do not use if you are pregnant, think you might be pregnant, or are breastfeeding (may harm an unborn baby). Do not use if you have a history of breast, uterine, or ovarian cancer, or if you have unexplained vaginal bleeding. Do not use if you have ever had a blood clot in your legs, lungs, or eyes, or have a clotting disorder. Avoid if you have active liver disease or severe liver impairment. Not for use in children or adolescents. Do not use if you are allergic to estradiol or any tablet ingredients.

May interact with seizure medicines (phenytoin, carbamazepine), rifampin, St. John’s wort, and some HIV medicines, lowering estradiol levels. Can increase effects of corticosteroids and theophylline. May change how blood thinners like warfarin work, so your INR may need closer checks. Thyroid replacement doses may need adjustment. Avoid grapefruit and grapefruit juice; they can raise estradiol levels and side effects. Always tell your provider and pharmacist about all prescription drugs, over-the-counter products, vitamins, and herbs you use.

Very common (more than 1 in 10): breast tenderness, nausea.

Common (1–10 in 100): headache, bloating, leg cramps, mood changes, mild weight gain.

Uncommon (1–10 in 1,000): high blood pressure, gallbladder problems, fluid retention causing ankle swelling.

Rare but serious: blood clots, stroke, heart attack, breast or uterine cancer. Seek urgent care for sudden chest pain, shortness of breath, vision changes, or severe leg pain. Most mild side effects improve within a few weeks; contact your provider if they persist or worsen.

Store at room temperature (68-77°F, 20-25°C) in the original blister pack. Keep tablets dry and away from heat, moisture, and direct light. Do not store in bathrooms or near sinks. Keep out of reach of children and pets. Dispose of expired tablets through a pharmacy take-back program or mix with coffee grounds in a sealed plastic bag before placing in household trash.

Frequently Asked Questions

1

How do I take a rapid dissolve tablet if my mouth is dry?

Place the tablet on your tongue and close your mouth. It should melt within seconds. If your mouth is very dry, sip a small amount of water first, then place the tablet.

2

When will I feel better after starting estradiol?

Many people notice fewer hot flashes in about 1–3 weeks. Vaginal dryness and sleep problems can take 4–8 weeks to improve. Full benefits may take up to three months of daily use.

3

Can I drink coffee or eat right after taking the tablet?

Yes. Once the tablet has fully dissolved and you have swallowed, you may eat or drink normally. If possible, wait a minute or two to be sure the medicine is absorbed.

4

Is it safe to smoke or vape while on estradiol?

Smoking, including vaping nicotine, increases the risk of blood clots, stroke, and heart attack when using estrogen therapy. Quitting lowers these risks. Talk to your provider about help to stop smoking.

5

What should I do if I get unexpected vaginal bleeding?

Call your healthcare provider right away. Although light spotting can occur early in therapy, unexpected or heavy bleeding needs evaluation to rule out more serious causes.

6

Can men or children use this medication?

No. These tablets are made for post-menopausal hormone replacement in adult women. They should never be used by men, children, or teens.

7

Do I need regular check-ups while on estradiol?

Yes. Your provider will likely want to see you every 6–12 months for blood pressure checks, breast exams, and possibly pelvic ultrasounds or mammograms. Keeping these appointments helps monitor safety.
