Use exactly as directed by your healthcare provider. Inject the medicine under the skin once each week on the same day. If you forget a dose and it is within 5 days of your usual day, take it as soon as you remember. If more than 5 days have passed, skip the missed dose and take your next dose on the regular day. Never take two doses at the same time.

Semaglutide attaches to GLP-1 receptors in your pancreas, brain, and stomach. This boosts insulin release when blood sugar is high, lowers the hormone glucagon, slows how fast food leaves the stomach, and reduces appetite signals in the brain. Blood sugar may improve within the first week, while noticeable weight loss often begins after 4-8 weeks and continues with ongoing use. Each dose works for about seven days because the medicine stays in the body a long time.

Do not use if you have or a family member has medullary thyroid cancer or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2) because the medicine may raise thyroid tumor risk. Do not use if you are pregnant, planning pregnancy, or breastfeeding. Do not use if you are allergic to semaglutide or any of its ingredients. Not for use in children under 18 years old. Do not use during episodes of diabetic ketoacidosis.

Using semaglutide with insulin or medicines that raise insulin, such as sulfonylureas (glipizide, glimepiride), can cause low blood sugar. Tell your provider about all diabetes medicines so doses can be adjusted. Semaglutide may slow the emptying of your stomach, which can change how quickly oral medicines like birth control pills, antibiotics, or thyroid pills are absorbed; take them at least one hour before the shot if possible. Avoid taking other GLP-1 medicines (liraglutide, dulaglutide) at the same time. Heavy alcohol use can increase stomach side effects and risk of low blood sugar. Check with your provider before starting new vitamins, herbs, or over-the-counter drugs.

Very common (more than 1 in 10 people): nausea (up to 44%), diarrhea, vomiting, constipation. These stomach issues often improve within a few weeks as your body adjusts. Common (1-10 in 100 people): stomach pain, burping, tiredness, headache, injection-site redness or itch, mild low blood sugar when used with other diabetes drugs. Uncommon (1-10 in 1000 people): gallstones, pancreatitis (severe belly pain that will not go away), worsening retinopathy in people with long-standing diabetes, allergic reactions like rash or swelling. Rare but serious: kidney problems from dehydration, thyroid tumors (lump in neck, hoarseness, trouble swallowing). Call your provider right away if you have severe stomach pain, yellow skin or eyes, a lump in your neck, or signs of an allergic reaction. Most mild side effects lessen over time or after dose adjustment.

Store unopened vials in the refrigerator at 36-46°F (2-8°C). Do not freeze; throw away if the medicine has been frozen. Once first used, the vial may be kept in the refrigerator or at room temperature below 86°F (30°C) for up to 28 days. Keep the vial in the original box to protect it from light and moisture, and keep the rubber stopper clean. Do not shake. Discard any leftover medicine 28 days after first puncture or after the expiration date, whichever comes first. Dispose of used needles in a sharps container and bring expired or unused medicine to a pharmacy take-back program or follow local disposal rules.
