Use as directed by your healthcare provider. Take your dose at the same time each day on an empty stomach with a full glass of water. If you miss a dose, take it as soon as you remember unless it is close to your next dose. Do not double up on doses. Your healthcare provider will decide how long you should stay on this medication.
Liothyronine (T3) and levothyroxine (T4) replace the thyroid hormones your body is missing. T4 is converted in your tissues to T3, the active hormone. T3 works inside cells to control metabolism, body temperature, heart rate, and energy use. Because this capsule releases the hormones right away, blood levels start to rise within 1-2 hours. Symptom relief—such as more energy and less brain fog—often begins within 1-2 weeks, but full benefits may take 4-6 weeks as hormone levels steady.
Do not use if you have untreated overactive thyroid (hyperthyroidism) or a recent heart attack. Do not use if you have uncorrected adrenal gland problems because it can trigger adrenal crisis. Do not use if you are allergic to liothyronine, levothyroxine, or any capsule ingredients. Tell your provider if you are pregnant; your dose may need adjustment but therapy is usually continued. Not for use as a weight-loss drug in people with normal thyroid function.
Thyroid hormones may raise the effect of blood thinners like warfarin (Coumadin) and increase bleeding risk. Seizure medicines (phenytoin, carbamazepine) and some antidepressants can speed up hormone breakdown, lowering levels. Calcium, iron, magnesium, and antacid products can block absorption—take them at least 4 hours apart. Cholestyramine and colesevelam can also reduce absorption. Estrogen therapy or birth-control pills may raise binding proteins and change your dose need. Tell your provider about all prescription drugs, over-the-counter medicines, and supplements you use.
Common (may affect up to 1 in 10 people): mild tremor, nervousness, trouble sleeping, headache, increased sweating. Less common (1 in 100): fast heartbeat, palpitations, diarrhea, weight loss. Rare but serious: chest pain, severe shortness of breath, irregular heartbeat, or signs of allergic reaction (rash, swelling, trouble breathing). Side effects often improve after dose adjustment. Call your provider right away if you have chest pain, rapid heartbeat, or severe anxiety.
Store at room temperature 68-77 °F (20-25 °C) in a dry place away from light. Keep capsules in the original closed bottle. Avoid bathroom or kitchen areas with high moisture or heat. Do not freeze. Keep out of reach of children and pets. Check the pharmacy label for expiration date and dispose of unused capsules through a drug take-back program or mix with coffee grounds in a sealed bag before discarding.
