Use exactly as directed by your healthcare provider. Swallow the capsule with a full glass of water, with or without food, at the same time each day. If you miss a dose, take it as soon as you remember unless it is within 8 hours of your next scheduled dose. Do not take two doses at once. Your healthcare provider will decide the right strength and length of therapy.
Liothyronine is a synthetic form of triiodothyronine (T3), one of the two main thyroid hormones. T3 enters cells and binds to thyroid hormone receptors in the nucleus, turning on genes that control energy use, heart rate, body temperature, and growth. Because this is the active hormone, it starts working within a few hours, and most people feel more energy or improved mood in 1–2 weeks. The E4M slow-release base spreads absorption over 8–12 hours, helping to avoid peaks and crashes and keeping blood levels steadier through the day.
Do not use if you have untreated or uncontrolled adrenal gland problems (adrenal insufficiency) because it can trigger a dangerous hormone imbalance. Do not use if you are experiencing uncontrolled overactive thyroid (thyrotoxicosis) or have a recent heart attack, as extra thyroid hormone can strain the heart. Do not use if you are allergic to liothyronine, thyroid hormones, or any capsule ingredients. Use with caution in pregnancy; only adjust doses under medical supervision. Not for weight-loss or bodybuilding purposes.
Liothyronine can increase the effect of blood thinners such as warfarin, raising bleeding risk—your doctor may need to adjust your dose. It may lower levels of digoxin, so heart patients on digoxin should have levels checked. Diabetes medicines (insulin, metformin, sulfonylureas) may need adjustment because thyroid hormone can change blood sugar control. Estrogen therapy or birth control pills can bind thyroid hormone and may require a higher liothyronine dose. Seizure medicines (phenytoin, carbamazepine) and rifampin may speed up liothyronine breakdown. Take antacids, iron, calcium, or cholestyramine at least 4 hours apart because they block absorption. Limit high-fiber supplements close to your dose for the same reason.
Very common (more than 1 in 10 people when dose is too high): rapid heartbeat, feeling shaky, sweating, anxiety, trouble sleeping. Common (1–10 in 100): mild weight loss, diarrhea, headache, heat intolerance. Uncommon (1–10 in 1,000): chest pain, shortness of breath, muscle weakness. Rare but serious: irregular heart rhythm, severe hypertension, bone loss with long-term high doses. Side effects usually improve within days after dose is lowered. Call your provider right away if you have chest pain, severe palpitations, or signs of allergic reaction (rash, swelling, trouble breathing).
Store at room temperature 68-77°F (20-25°C). Keep capsules in their original tightly closed container, protected from moisture and direct sunlight. Do not store in the bathroom or kitchen. Keep out of reach of children and pets. Dispose of unused or expired capsules through a pharmacy take-back program or mix with coffee grounds in a sealed bag before throwing in household trash.

Many people feel a boost in energy or mood within 1–2 weeks, but full benefits for symptoms like brain fog or weight gain can take 4–6 weeks. Your provider will check blood tests and adjust your dose for the best effect.
Should I take my capsule with food or on an empty stomach?
You can take it with or without food, but try to take it the same way every day. Avoid high-fiber meals, calcium, or iron supplements within 4 hours of your dose because they can lower how much medicine you absorb.
Can I drink coffee or tea after taking this medicine?
Wait at least 30 minutes before drinking coffee, tea, or other caffeinated drinks. Caffeine may speed up stomach emptying and reduce absorption if taken too soon after the capsule.
Is liothyronine safe during pregnancy?
Thyroid hormone is important for the baby’s brain growth, so treatment should not be stopped. Your doctor may switch you to levothyroxine or adjust the liothyronine dose and will check your labs often to keep levels in the safe range.
What if I accidentally take two doses on the same day?
Skip the next scheduled dose and call your healthcare provider if you feel fast heartbeat, nervousness, or chest pain. Do not try to ‘balance’ the mistake by skipping more doses unless your provider tells you to.
Will this medication help me lose weight?
Liothyronine is not a weight-loss drug. It is meant to replace thyroid hormone in people who need it. Taking more than prescribed can be dangerous and may harm your heart and bones.
